Page 3 contains a list of tests that can or should be provided to our members on an annual wellness check up with your PCP. The Wellness Team asks that you complete your physical prior to coming for your annual DFD Wellness Screening. Having the lab work at this visit will add tremendous value to your annual Wellness Screening.
